Vagifem (estradiol) is a localized estrogen replacement therapy typically prescribed for menopausal women.

While the active ingredient, estradiol, may be used in a range of estrogen replacement therapies, Vagifem focuses on treating uncomfortable vaginal side effects, sometimes called vaginal atrophy or atrophic vaginitis, which many women experience as estrogen levels drop. Symptoms include:

• Vaginal dryness, itching or burning
• Pain or bleeding during intercourse
• Painful or urgent urination

As the name implies, Vagifem vaginal tablets are inserted into the vagina. Estrogen released is then absorbed by surrounding tissues; a highly localized effect. Do not buy Vagifem to treat estrogen deficiencies elsewhere in the body; it will have little effect.

Proper Use of Vagifem

Vaginal tables come in strengths of 10 to 25 mcg. For the vast majority of women 10 mcg is ideal; in rare cases stronger doses may be recommended.

Treatment usually begins by applying 1 tablet daily for 2 weeks, after which application is reduced to 2 tablets each week. This may vary depending on individual circumstances, however. In all scenarios treatment is most effective if administered on schedule; at the same time each day, on the same days of the week.

Application is similar to using a tampon:

• Each vaginal tablet comes packaged in a single-use applicator.
• Wash and dry hands before handling applicator.
• Insert applicator into vagina and press plunger.
• Discard used applicator.

Duration of treatment may vary widely, depending on individual needs. In general it is considered better to limit hormone replacement of any kind to as low a dose for as brief a time as possible, but in some situations the benefits of higher doses or long-term treatment may outweigh the negative risks.

It is also possible to buy Vagifem as a vaginal cream or vaginal ring, for those women who find vaginal suppositories less than ideal.

Precautions & Side Effects 

Estrogen levels can have a tremendous influence on bodily function and health. Often other hormones, namely progestins, are prescribed with estrogen replacement to reduce the risk of severe side effects. Physician instructions should be followed carefully for the duration of treatment.

The effectiveness of treatment may be reduced by other medications and supplements. Check with a doctor or pharmacist before mixing medications, and be sure any doctor providing care is aware of the treatment.

Likewise any major medical condition may impact treatment effectiveness, or increase the risk of side effects. Women who are pregnant or breast feeding should not buy Vagifem; estrogen may interfere with the child's development. 

In otherwise healthy women, side effects are generally mild and often pass as the body becomes used to treatment:

• Nausea, loss of appetite
• Swollen, tender breasts
• Headache or dizziness
• Spotting
• Acne or skin changes
• Swelling of lower extremities

Women who have or at high risk of developing breast cancer, uterine cancer or blood clots should avoid estrogen supplements, as the treatment may exacerbate these conditions. There is also some evidence that estrogen use, either alone or with a progestin, may increase risk of developing dementia.
